The Quality Care Manager is responsible for reviewing patient assessments to ensure appropriate and effective care is provided to each patient in compliance with agency policies, procedures, and regulatory requirements. Trends or patterns of inaccuracy identified by the QCM will be communicated to the Quality Director to assist in the development of the performance improvement and education plan. Home Care Home Base (HCHB) preferred. Pay range (depending on experience): $80-95k annual salary Schedule: M-F States eligible to hire: AZ, FL, IA, ID, IL, MS, NC, NV, OR, SC, TN, TX, UT, VA, WA
Must be a graduate of an accredited School of Nursing Current unencumbered license to practice as a Registered Nurse (RN) Minimum of 1 year of home health experience with a working knowledge of Medicare Home Health Conditions of Participation, and OASIS Excellent written and oral communication skills required. Ability to handle stressful situations/deadlines. Strong computer and software skills with knowledge of Home Care Home Base highly preferred. Self-starter, goal directed with ability to work independently and stay focused without on-site supervision. Ability to communicate effectively with agency staff, upper management, and other members of the health care team.
Communicates effectively with all members of the health care team and maintains an effective collaborative relationship with the leadership team as well as other departments. Performs review of documentation and plan of care to ensure congruency with primary focus of care, physician face to face encounter, homebound status (if required) including utilization of appropriate disciplines. Confirms all documentation submitted to workflow is accurate and complete and sufficient to support skilled need and medical necessity. Reinforces agency processes when offering feedback to clinicians regarding documentation deficiencies or OASIS corrections. Make sure visit utilization is supported by individual patient assessment. Reviews additional orders for appropriateness. Confirms clinical documentation supports Recertification/Discharge decision. Approves the final Plan of Care (485) timely, confirming it is complete, accurate, and individualized to the patientās needs. Reviews all OASIS assessments for inconsistencies and locks timely.
