As a Chronic Care Manager, you will embody the essence of compassionate care, providing education and support tailored to each patient’s unique needs. Flexible Work Environment: Enjoy the benefits of a remote role with customizable schedules that cater to your lifestyle while fostering meaningful patient relationships. Work Schedule: Monday to Friday, with options of 8:00 AM - 4:30 PM EST or 11:00 AM - 7:30 PM EST. After the initial six-month period, you may choose a work schedule that best fits your needs: 4 days x 8 hours 4 days x 9 hours 4 days x 10 hours Your benefits will remain consistent regardless of your schedule.

Conduct regular check-ins with patients to monitor their health status and address any concerns. Build and maintain trusting relationships with patients and their families. Serve as a critical link within the patient’s care team, facilitating communication with healthcare providers. Leverage your clinical expertise to guide patients towards better health management. Utilize technology to coordinate care efficiently and set achievable health goals.
