Our niche area is in the Managed Care Industry where we work closely with various Managed Care clients across the country to support their mission of delivering quality services to their member population while containing costs. Our clinicians are experienced in Case Management, Care Coordination, Needs Assessments, Medication Adherence Counseling, Post Hospital Assessments, Medication Reconciliation and/or Level of Care Determinations. These clinicians are available to work on a consulting, permanent, temp to permanent or per diem basis. Grace Care Consulting is certified in Pennsylvania as a Veteran Owned Service Disabled Small Business.
