Our team has created proprietary software that is used to track patients’ physiological values (Blood Pressure, Weight, Blood Glucose, Lung Capacity, Oxygenation). The software is used to identify out-of-range values that can be dangerous, and to notice trends and/or triggers that may be impacting an individual's medical condition. To follow up on patients over time and offer a more personalized quality of care, an RPM Healthcare Care Coach is assigned to support, track and analyze patients’ data and engage patients in their health by providing guidance, education and encouragement. We are adding Chronic Care Management into our platform
