Provide education, counseling, and support to patients and caregivers regarding chronic disease management, medication adherence, and lifestyle changes. Document all patient interactions accurately in the electronic health record (EHR) system. Collaborate with physicians, advanced practice providers, and interdisciplinary teams to ensure continuity of care. Respond promptly to patient inquiries and follow up on care plans or referrals. Utilize clinical judgment and established guidelines to escalate cases when needed. Participate in continuous quality improvement and telehealth best practices development.
