Nurse Care Manager (Fully Remote - Adult Care Management) Included Health | 6-Month Contract-Hire (Possible Extension) Start Date: January 5, 2026 (Must be hired by 12/15) About the Role Included Health is seeking passionate Nurse Care Managers to join our Care and Case Management team. This role is ideal for nurses who truly enjoy providing holistic, patient-centered support throughout the healthcare journey. As a telephonic Nurse Care Manager, you will guide members through complex medical and behavioral health needs, offering compassionate care, clinical insight, and coordinated support. You will work alongside a multidisciplinary clinical team-including care coordinators, records specialists, and other healthcare professionals-to deliver integrated, remote care in a highly innovative environment. This role is best suited for nurses who excel in telephonic communication, care planning, empathy, and navigating medical benefits and healthcare systems. Details Monday-Friday, 9:00 AM - 6:00 PM PST Monday-Friday, 9:00 AM - 6:00 PM local time Schedule Depends on openings - Need to be comfortbale working PST if required Training: Monday-Friday, 10:00 AM - 7:00 PM EST for two weeks No time off permitted during January Start Date: 1/5/2026 Compensation: $40-$42 / Hour Location: Remote
Qualifications (Required): Bachelor of Science in Nursing (BSN). Active Compact RN license in good standing. 2+ years of experience in adult Care Management (Case Management, Disease Management). 5+ years of clinical nursing experience. Must reside in an NLC Compact state. Remote care/case management experience required. Ability to work until 6 PM PST (preference for MST/PST candidates). Comfortable working with multiple EMR and computer systems. Strong verbal and written communication skills. High empathy and excellent clinical judgment. Preferred Qualifications: Active California RN license (not required). Experience in a health plan, health navigator, or TPA environment. Spanish-speaking abilities. Experience working fully remote. Strong grammar, detail orientation, and ability to write patient-friendly medical summaries. Soft Skills: Highly empathetic and supportive. Efficient and productive while meeting volume expectations. Adaptable in a rapidly changing, fast-growing environment. Strong prioritization and critical-thinking skills. Collaborative, pleasant, and eager to learn. Excellent communicator across disciplines and patient populations.
Deliver coordinated, patient-centered virtual Care Management through phone and/or video. Create comprehensive, person-centered care plans alongside members and the clinical care team. Support members as they navigate complex medical conditions, treatment pathways, benefits, and systems. Partner with local providers to ensure continuity and alignment of care. Provide consistent, compassionate follow-up and long-term support. Manage acute episodes, including hospitalizations and rehab stays, through coordinated Case Management. Identify clinical and social resource needs and coordinate holistic support. Maintain strong communication with members, families, and colleagues. Strictly adhere to HIPAA and all security protocols.
