This position is responsible for conducting face-to-face assessments of members who may need skilled nurse assessment services. The incumbent completes all assessments using a skilled care acuity tool and provides a recommendation for services based on members health care needs. Members who are assessed reside in the community or are ready to be discharged from a facility. The incumbent may also serve as mentor or preceptor for new staff, assisting in the formal training of assessors, complete quality assurance audits, inter-rater-reliability reviews, and may be involved in process improvement initiatives.
Four-year nursing degree and at least three years of experience in nursing. Experience working with individuals with disabilities, individuals with intellectual and developmental disabilities (IDD). Experience working with Pediatric and Geriatric populations with various disabilities, and individuals with complex health needs preferred. Current and unrestricted RN licensure in Colorado or a compact nursing license preferred. Bilingual or Spanish speaking preferred. This individual will be expected to have occasional travel to surrounding areas.
Performs face-to-face and/or virtual assessments of persons with disabilities. Collaborates with health care providers and ancillary service providers to create or validate the appropriate treatment plan. Documents assessment results for the individual member status or negotiates service plan as needed. Completes quality assurance competency reviews and other tasks/projects as assigned such as new assessor training. Provides ongoing education. May communicate with all health care providers to ensure appropriateness of care. Performs miscellaneous duties as assigned.
