We're a dedicated team of cardiology clinicians, operators, and technologists improving patient outcomes and experience by increasing access to quality cardiac care via our virtual care team and cardiac device agnostic platform. We are currently community cardiologists and their patients in California and Arizona, and are rapidly expanding. We're seeking passionate problem-solvers to tackle one of healthcare's most complex challenges in revolutionizing heart failure care. As a NP working on ISHI's multi-disciplinary clinical team you will work closely with HF MD's, nurses, HF NPs, and pharmacist to enable virtual cardiac care for our patients. Position: Nurse Practitioner Location: Remote Schedule: Full time Working hours : 8am - 5pm PST Monday through Friday

Deliver longitudinal virtual care to patients with cardiac conditions, including heart failure, hypertension, and cardiometabolic comorbidities, in collaboration with ISHI’s multidisciplinary team. Interpret and act on device data and clinical alerts (e.g., weight, PAD, BP, heart rate trends), using ISHI’s AI-driven platform to guide timely interventions and escalations. Conduct virtual visits for medication titration, symptom management, and care coordination with PCPs and cardiologists, with an emphasis on optimizing guideline-directed medical therapy (GDMT). Support post-hospital discharge transitions, identifying gaps in care and reducing readmission risk through structured check-ins and escalation pathways. Document encounters and interventions clearly in ISHI’s digital tools and ensure timely communication with referring providers and care teams. Participate in care team huddles and quality reviews, contributing clinical insight to help refine ISHI’s programs and protocols. Educate patients and caregivers on heart failure self-management, medication adherence, and lifestyle changes using virtual tools and remote monitoring insights.
