About Companian Companian is building the wellness companion for older adults. We pair advocates that are supported by AI with seniors — coordinating care, navigating the healthcare system, and keeping families in the loop. The U.S. healthcare system leaves older adults and their families to figure out the hard parts alone, and primary care practices aren't financially set up to fill the gap. Companian fills it. We're early, moving fast, and building something that genuinely improves how older adults age in place. About the Role As a Care Advocate at Companian, you'll be the human in the loop for a panel of older adults. You'll work alongside our AI companion, translating those signals into action. When a patient is confused about a new prescription or needs help scheduling an appointment, you're the person who reaches out, figures out what's going on, and makes sure it gets handled. You'll navigate members through complex and everyday health concerns, address social determinants of health (SDOH), and serve as a trusted point of contact for both members and their families. Time Commitment & Compensation This is a 1099 contractor role, fully remote. You bill by the hour for time spent on member work — calls, coordination, research, and documentation — and log your hours in the Companian platform. • Minimum commitment: 8–10 hours per week. There is no maximum; advocates who want more volume can take on a larger panel. • Availability during business hours is required. Providers' offices, pharmacies, and insurers are only reachable during the standard workday, and so are most of our members. You'll need a consistent block of availability at some point between 8:00am and 5:00pm ET, Monday through Friday. You choose which hours — but they need to fall inside that window and stay reasonably predictable week to week. • Evening and weekend work is not required. What You'll Do • Own ongoing relationships with a panel of older-adult members, building trust over time as their dedicated advocate. • Monitor check-in signals from the Companian platform and act on what's "off" — escalating, following up, and closing the loop. • Develop and maintain care plans that holistically address SDOH: food access, transportation, in-home support, social isolation, and caregiver strain. • Coordinate care across providers — scheduling appointments, clarifying treatment plans, helping members understand bills and coverage, and bridging gaps between PCPs, specialists, and the member. • Log your time accurately against each member's record • Provide fearless feedback to the founding team. You'll directly shape how the advocacy model and product work as we build and scale. What We're Looking For • Professional experience as a Social Worker, Care Coordinator, Case Manager, Nurse, or similar patient-facing role. • Availability for at least 8–10 hours per week, with a predictable block falling between 8:00am and 5:00pm ET on weekdays. • Deep understanding of Social Determinants of Health and experience working with older adults and diverse patient populations. • Strong knowledge of how to navigate the U.S. healthcare system on a patient's behalf — appointments, referrals, coverage, and care transitions. • Endless empathy, strong judgment, and the drive to fight for people who can't always fight for themselves. • Comfort working autonomously in an early-stage, ambiguous environment. • Excellent written and verbal communication. Nice to Have • MSW, LCSW, LMSW, LBSW, LPN, case management experience at a hospital, skilled nursing facility, or rehab center. • Experience in geriatrics, home health, hospice, hospital case management, or Medicare/Medicare Advantage populations. • Bilingual (English/Spanish) — strongly preferred for our South Florida members. • Prior experience at an early-stage or tech-enabled healthcare company.
