About Companian Companian is building the wellness companion for older adults. We pair advocates that are supported by AI with seniors — coordinating care, navigating the healthcare system, and keeping families in the loop. The U.S. healthcare system leaves older adults and their families to figure out the hard parts alone, and primary care practices aren't financially set up to fill the gap. Companian fills it. We're early, moving fast, and building something that genuinely improves how older adults age in place. About the Role As a Care Ambassador at Companian, you'll be the clinical front door for a panel of older adults — many of them newly discharged from nursing homes and skilled nursing facilities, at one of the most fragile moments in their care journey. Your core focus is conducting intakes: completing the clinical assessments that get each new member set up for success from day one, as a licensed NP in Tennessee or Florida. You'll work alongside our AI companion and our Care Advocates, turning the signals they surface into sound clinical decisions. When a member's symptoms change, a medication needs a second look, or a care plan needs a clinician's judgment, you're the person who steps in, makes the call, and makes sure it gets handled. You'll deliver virtual care to seniors, guide complex and everyday health concerns, and serve as a trusted clinical voice for members, their families, and the team building around you. This is proactive, relationship-driven care — not reactive, high-volume churn. This is a 1099 role. You can work part-time or full-time, remotely. What You'll Do - Conduct intake assessments for newly enrolled members — many transitioning home from nursing homes and skilled nursing facilities — getting each member's care started on the right footing. - Provide telehealth care to a panel of older-adult members, building trust over time as a steady clinical presence in their lives. - Act on check-in signals from the Companian platform and Care Advocate escalations — assessing, diagnosing, and closing the loop. - Develop and oversee care plans, managing acute and chronic conditions within your scope of practice. - Collaborate with supervising/collaborating physicians, Care Advocates, and other providers — clarifying treatment plans, supporting care transitions, and bridging gaps between PCPs, specialists, and the member. - Provide fearless feedback to the founding team. You'll directly shape how the clinical model and product work as we build and scale. What We're Looking For - Active, unrestricted Nurse Practitioner license in Tennessee and/or Florida, in good standing. - Board certified (e.g., FNP, AGNP, or Geriatric NP). - Active collaborative practice or supervising physician agreement in place where required by state law, or willingness to establish one. - Comfort practicing in a telehealth-first environment, fluent with the technology that care now runs on. - Strong clinical judgment and the drive to fight for people who can't always fight for themselves. - Comfort working autonomously in an early-stage, ambiguous environment. - Excellent written and verbal communication. Nice to Have - Experience caring for a senior or older-adult population — geriatrics, home health, hospice, hospital case management, or Medicare/Medicare Advantage populations. - Experience in digital health, telehealth, or virtual care delivery. (Either this or senior-care experience is enough — both is even better.) - Prior experience at an early-stage or tech-enabled healthcare company.
