Care Manager AIM RN

We are looking for a Remote RN Care Manager for our AIM (Advanced Illness Management) Program covering our Toledo, OH palliative care branch. This position will directly report to the Director of Care Management AIM and is responsible for working with patients, their families, PCP, IDT, and other health care professionals to provide and coordinate high-quality end of life care. Ohio State RN Licensure Required Candidate must live in the state of Ohio.
