The position is remote, based in Philadelphia, PA, and is a contract opportunity. The Care Manager will engage with members to support self-management and coordinate care effectively.

Function as a primary nurse for members across various health services. Engage with members to develop and support self-management abilities. Provide clinical coaching support for acute symptoms and chronic conditions. Conduct assessments to identify needs and develop care plans. Facilitate communication between patients, families, and healthcare teams. Identify resources to assist members in achieving health goals. Ensure compliance with regulatory requirements and accreditation standards.
