The position is remote, based in Philadelphia, PA, and is a contract opportunity. The Care Manager will engage with members to support self-management and coordinate care effectively.
Current, active, and unrestricted Pennsylvania Registered Nurse licensure. Three years of clinical, case management, and/or disease/condition management experience. Exceptional communication and problem-solving skills. Ability to work flexible hours. Basic computer skills. Function as a primary nurse for members across various health services. Engage with members to develop and support self-management abilities. Provide clinical coaching support for acute symptoms and chronic conditions. Conduct assessments to identify needs and develop care plans. Facilitate communication between patients, families, and healthcare teams. Identify resources to assist members in achieving health goals. Ensure compliance with regulatory requirements and accreditation standards.
