Case Manager RN

Position:- RN Case Manager Rate:- $40/hr. Location :- States: Colorado, Georgia, Indiana, Kentucky, Mississippi, Ohio, Pennsylvania, Virginia, Washington. Job Description:- Case Manager RN – The Case Manager RN coordinates a multidisciplinary team to deliver holistic, person-centered care to a diverse health plan population. Serving as the primary contact for members, caregivers, and providers, the RN assesses, develops, implements, and monitors individualized care plans to improve health outcomes across the care continuum.

Lead coordination with medical, behavioral, pharmacy, dietitian, and support staff. Assess member needs (health, psychosocial, cultural) and develop care plans. Arrange and coordinate services (community, mental health, financial, DME, transportation). Educate on health literacy, self-management, and post-discharge care. Monitor and adjust care plans to ensure progress toward goals. Document all interactions accurately and meet quality and productivity standards.
