Skills • Current Licensed Practical Nurse (LPN) license with 1.5 to 12 years of experience • Strong problem-solving skills with the ability to multitask effectively • Excellent verbal and written communication skills • A customer-service oriented mindset, always putting patients’ needs first • Bilingual (English/Spanish) is a plus but not required • Experience using EHR/EMR systems to manage patient data is preferred Responsibilities • Work collaboratively with a care team to improve the health and well-being of chronic care patients • Utilize the Chronic Care Management platform to coordinate care for patients managing multiple chronic conditions • Organize and analyze patient data to identify gaps in care and unmet needs • Assist healthcare providers in creating and implementing care plans and interventions for patients • Manage a patient panel (approximately 500 patients, with 300 billable by the end of the month) • Provide proactive outreach and support to patients, ensuring seamless communication between the patient, their care team, and any other necessary services • Contribute to continuous improvement initiatives aimed at enhancing the quality and efficiency of care • Attend team meetings and engage in professional development opportunities • Ensure compliance with all relevant healthcare regulations and organizational policies Qualifications • Current Licensed Practical Nurse (LPN) license with 1.5 to 12 years of experience • Experience working in population health or chronic care management is a plus • Excellent verbal and written communication skills Education Requirements • Current Licensed Practical Nurse (LPN) license Education Requirements Credential Category • Vocational or Associate Degree in Nursing Experience Requirements • 1.5 to 12 years of experience in nursing, preferably in population health or chronic care management Why work in Grand Prairie, TX Grand Prairie offers a unique blend of vibrant community life, with a range of recreational and cultural activities.
