Clinical Field Reviewer (Must Reside in Eastern Montana)

Mountain Pacific was established in Montana, in 1973. Mountain Pacific is a nonprofit organization that partners within our communities to provide solutions for better health. Through federal, state and commercial funding and partnerships, we bring national public health priorities to the local, state or regional level, leveraging our dynamic staff members’ wide range of expertise to advance better health and health care services.

We are a team of Registered Nurses working remotely across the state of Montana completing in-home nursing assessments of eligible members living in the areas where we live for the Medicaid CFCS/PCS (Community First Choice Services/Personal Care Services) program. We conduct person-centered in-home nursing assessments focused on a member’s functional abilities and their needs for ADL and IADL assistance initially and annually and as needed if a change in condition occurs for members of all ages. We have a shared goal of keeping members independent in their homes with the assistance of paid caregivers and informal support. We work closely with provider agencies across the state to ensure that services are being delivered to members as authorized following policies set forth by the State of Montana DPHHS. We provide education, empower members to become participants in their care through active listening and planning with their input, provide care coordination, practice cultural humility with each client, and utilize evidence-based practice as a foundation for our interventions.
