Clinical Supervisor - RN

Aims healthcare is a San Ramon based home health agency serving patients all over bay area and surrounding counties.

We are seeking a dedicated and experienced Nursing Supervisor to oversee the nursing as well as therapy staff and ensure high-quality patient care within our facility. The ideal candidate will possess a strong background in home health environment, experience as field nurse case manager. This role requires excellent leadership skills, a commitment to patient safety, and the ability to manage complex medical situations effectively.

Supervise staff and oversee coordination of patient care activities to ensure adherence to best practices and regulatory standards. Conduct assessments of patient needs and develop individualized care plans in collaboration with healthcare teams. Ensure accurate medical documentation and compliance with California title-22 and with ICD-9 coding for billing purposes. Facilitate communication between patients, families, and healthcare providers to enhance patient satisfaction and outcomes. Provide training and support for nursing staff on laboratory procedures, medication administration, and emergency response protocols. Provide support to clinicians and back office staff for supply orders and follow ups. Monitor patient progress and adjust care plans as necessary to meet changing health needs.
