Conduct thorough patient assessments, including vital signs, medical history review, and physical examinations using EMR (Electronic Medical Record) systems such as Epic or Cerner. Provide expert nursing care for patients requiring complex interventions such as IV infusions, catheterizations, wound suturing, ventilator management, and ostomy care. Collaborate with physicians and healthcare teams to develop and implement individualized care plans for diverse populations including geriatrics, pediatrics, behavioral health patients, and those in hospice or palliative settings. Assist with triage and urgent care evaluations remotely, determining priority levels for patients presenting with acute pain, infectious diseases, or trauma-related concerns in Level I or Level II trauma center scenarios. Support long-term care facilities and assisted living communities by managing medication administration, documentation compliance (including ICD-10 coding), and conducting routine laboratory specimen collection or processing. Educate patients and families on health management strategies such as nutrition, medication adherence, infection control practices, and post-discharge instructions to promote optimal recovery at home or in community settings. Maintain HIPAA compliance by safeguarding patient information during all documentation and communication activities while ensuring accurate medical records updates.
