Job description

To support palliative care needs, the full-time remote Georgia Licensed Palliative Care LPN will conduct telephonic assessments, educate patients and families, and facilitate transitions to hospice care while collaborating with clinical teams. Key responsibilities • Conduct telephonic palliative assessments and determine next steps, including scheduling visits and arranging hospice referrals • Complete hospice informational sessions with patients and families, providing education on hospice benefits and eligibility criteria • Document in real time using EMR systems and support care transitions through accurate documentation Required qualifications • Current and unencumbered LPN license in Georgia • 1+ years of hospice or palliative care experience in a patient-facing role • Knowledge of Medicare hospice regulations and managed care • Proficiency in Microsoft Office and general computer skills • Experience in task triage and time management in a remote environment
