Cadence is seeking a dedicated and experienced Registered Nurse with a background in managing patients with congestive heart failure (CHF) and inpatient care. This role involves collaborating closely with clinical teams to support chart setup, enhance patient engagement, and provide targeted education to improve patient outcomes. The ideal candidate will have strong clinical expertise in CHF, excellent communication skills, and a passion for patient-centered care.
RN compact multi-state license and CA license required, and a willingness to expand state licensure if needed as Cadence adds new markets. 5+ years of experience treating patients with chronic conditions including CHF, Hypertension, and/ or Diabetes patients either in an outpatient or inpatient setting. Passion for the patient/customer experience and systematically improving healthcare with digital innovation. Independent thinker/operator (comfortable working in a less structured environment). Ability to thrive in an environment founded on trust, autonomy and direct communication & feedback. Love of owning problems end-to-end. Independent thinker / operator. Passion for the patient / customer experience and systematically improving healthcare with digital innovation. Experience working in a CHF bridge clinic environment, T2D clinic (preferred). Experience working with remote patient monitoring technology (preferred). Experience working in a startup environment (preferred).
Assist in the setup and maintenance of patient charts, ensuring accuracy and completeness of clinical documentation related to CHF management and inpatient care. Collaborate with multidisciplinary teams to support seamless inpatient management and continuity of care. Deliver comprehensive patient education on CHF, treatment plans, medication adherence, lifestyle modifications, and self-management strategies. Facilitate patient engagement initiatives to promote active participation in their care and improve health literacy. Monitor and assess patient conditions, reporting changes promptly to the healthcare team. Support discharge planning and coordination with outpatient services to ensure smooth transitions of care. Participate in quality improvement activities related to CHF management and patient education programs. Maintain up-to-date knowledge of best practices in CHF care and inpatient nursing standards.
