Role Description The LPN Ambulatory Virtual/Telehealth Nurse will provide patient care using virtual platforms, demonstrating proficiency in telehealth technologies and clinical protocols. They will assess and manage patient needs, collaborate with providers, manage prescriptions, and complete prior authorizations. Strong time management, attention to detail, and cultural sensitivity are crucial in this role. Key Responsibilities: • Patient Care & Clinical Management: • Provide virtual nursing care using telecommunication technologies (telephone, video, chat, etc.). • Assess patient needs using clinical protocols, guidelines, and Synergy model for Care. • Assist with medication management and scheduling appointments. • Route medication renewal orders to providers for review per protocol. • Collaborate with patients regarding prescription management, including protocol orders. • Identify and report patient safety and quality issues. • Act as an advocate for the patient, supporting optimal health outcomes. • Educate patients on the provider's plan of care. • Resolve pharmacy/medication issues in collaboration with providers. • Operational Support: • Document, update, and maintain patient medical records. • Support clinical and administrative operations of telehealth. • Participate in staff meetings, huddles, and organizational meetings. • Train, orient, and educate new team members and students. • Maintain confidentiality in all interactions and follow telehealth policies. • Communication & Collaboration: • Serve as a liaison between patients and providers. • Demonstrate effective listening skills and conflict resolution abilities. • De-escalate calls and manage difficult situations. • Collaborate actively with the team and other health system partners. • Compliance & Professional Development: • Adhere to the ANA Code of Ethics for Nurses. • Maintain competencies for telehealth nursing. • Participate in professional development activities. • Meet minimum quality, safety, and productivity metrics consistently. • Technology: • Use Emory-approved telehealth platforms and technology in patient care delivery. • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and eMR. • Utilize TEAMS, email, and other communication platforms to stay informed. Qualifications • High School Diploma or GED required. • Graduate of an accredited Licensed Practical Nursing Program. • Previous nursing experience preferred, not required. • Valid, active, unencumbered License of Practical Nursing approved by the Georgia Board of Nursing. • An active compact/multistate license (eNLC) is required within 60 days of hire. • BLS certification. Physical Requirements • Light lifting (1-10 lbs): 0-33% of the work day (occasionally), negligible. • 34-66% of the workday (frequently), negligible. • 67-100% of the workday (constantly). • Lifting 10 lbs max, carrying of small articles such as dockets, ledgers, files, small tools. • Occasional standing & walking, frequent sitting, close eye work (computers, typing, reading, writing). Environmental Factors • Factors affecting environment conditions may vary depending on the assigned work area and tasks. • Environmental exposures include, but are not limited to: Blood-borne pathogen exposure, Bio-hazardous waste, chemicals/gases/fumes/vapors, communicable diseases, electrical shock, floor surfaces, hot/cold temperatures, indoor/outdoor conditions, latex, lighting patient care/handling injuries, radiation, shift work, travel may be required, use of personal protective equipment, including respirators.
