Homecare Assessment RN

We are currently seeking a dynamic and experienced homecare assessment RN to conduct assessments for our patients. The ideal candidate will have a strong background in home care and home health care. This position is currently per diem and paid per assessment and re-assessment, with potential to move to full-time. Most of the assessments will be done via Telehealth, but you may be required to conduct home visits within your borough on a case-by-case basis.

Conducts initial and regular assessments of patients in their homes or via Telehealth Develops individualized care plans based on patient needs, goals, and the findings of assessments Accurately documents UAS assessments, care plans, and nursing interventions Manage relationships with referral sources, community partners, and stakeholders to promote the organization’s services and expand patient access. Monitor and ensure compliance with documentation standards, including patient assessments, care plans, and regulatory requirements. Drive quality improvement initiatives and implement evidence-based practices to enhance patient outcomes. Collaborate with interdisciplinary teams to ensure coordinated care and seamless service delivery.
