Completes an initial, comprehensive and ongoing assessment of patients. Initiates the plan of care and makes necessary revisions as needs change. Completes, maintains and submits accurate and relevant clinical notes in HomecareHomebase (Pointcare) prior to end of shift. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Teaches the patient and family/caregiver selfcare techniques as appropriate. Other visits as needed based on your availability including routine visits, COC, GIP, CC, etc. Perform afterhours oncall visits during scheduled workdays
