Hospice Per Diem Triage RN

Triage RN (Per Diem) – Fully Remote Day Shift (8:30 AM–5 PM) or NOC (5 PM–8:30 AM) | Weekday + Weekend/ Holiday Availability Needed Calstro Hospice is looking for a compassionate, calm-under-pressure Triage RN to support our patients and families outside of standard business hours. If you love patient advocacy, thrive in a fast-moving environment, and prefer caring for patients remotely, this role is your sweet spot. You’ll be the clinical first-response for after-hours calls, providing telephonic assessments, guidance, and support to patients, caregivers, and facility partners. You’ll follow all CMS/state guidelines while helping maintain high-quality, patient-centered care. Take after-hours and weekend calls, offering clinical guidance and emotional support Assess patient needs over the phone and escalate appropriately Notify Case Managers and leadership of changes in patient condition Communicate with physicians and obtain needed orders Prioritize calls based on acuity and ensure timely follow-up Coordinate after-hours staff visits when needed Complete accurate clinical documentation after each call Support and guide On-Call LVNs as needed
