Job description

Job Overview We are seeking a self-motivated Intake Registered Nurse (RN) with a passion for cardiac care to join our rapidly expanding telemedicine practice. Applying your clinical expertise and knowledge, you will use appropriate clinical decision-making skills to deliver remote, evidence-based care. As the first clinical point of contact, you will conduct initial consultations and support the execution of individualized care plans based on current clinical standards and national guidelines, working under the guidance of our program lead NPs. You will serve as a vital clinical resource and educator, collaborating simultaneously with a care team of patient navigators and other RNs to move patients toward their targeted health goals, ensuring they have the resources and coordination needed for successful chronic care management from the comfort of their own homes. Key Responsibilities • Assess and confirm eligibility for newly enrolled patients in our network using outside medical records. • Perform initial intake visits of new patients within the cardiac programs. • Identify and coordinate which program best suits the needs of the patient and what devices are appropriate for their remote monitoring. (HF, HTN, AFib) • Provide patient education focusing on the management of chronic cardiac conditions. • Instruct patients on device usage and troubleshooting, ensuring they feel confident, supported, and empowered in their remote monitoring journey. • Engage and coordinate with the NPs as needed for risk assessment and modification of the treatment plans. • Document all patient interactions comprehensively within the organization's EHR system. Requirements • Active Registered Nurse (RN) license; compact/multi-state license preferred. • Minimum of 2 years of direct patient-facing clinical experience. • Technical aptitude with proficiency in EHR systems. • Attention to detail and excellent communication skills. This role requires most of your shift on the phone. • Ability to thrive in a fast-paced, sedentary, phone-based remote work environment. • A HIPAA-compliant workspace. • Active high-speed Wi-Fi. • Previous experience in a telehealth environment is preferred. • Being bilingual is a plus. Benefits & Schedule • Pay: Hourly • Schedule: Full-time: Monday - Friday (8:00 am - 4:30 pm CST or 9:00 am - 5:30 pm CST). No weekends, no holidays, and no on-call. Part Time: Tuesday-Thursday (8:00 am-4:30 pm CST) • Fully remote position with company laptop provided. • Comprehensive health benefits (medical, dental, and vision) and a 401k with a company match.
