About DayaMed DayaMed partners with primary care practices and provides direct to consumer telehealth services to improve patient outcomes through Chronic Care Management, Remote Patient Monitoring, and preventative care. As we continue expanding into additional states, we are looking for experienced nurses who are passionate about delivering exceptional virtual patient care while maintaining the highest standards of compliance and documentation. Position Summary The Remote LPN is responsible for providing telephonic and virtual clinical support for patients enrolled in CCM, RPM, and DTC programs. This position includes conducting monthly follow up calls, monitoring RPM alerts, assisting providers with patient care coordination, documenting patient encounters, and supporting workflows. This is a structured, process driven position that requires excellent communication skills, strong clinical judgment within the LPN scope of practice, and the ability to consistently follow established workflows while adapting to organizational growth and regulatory changes. Responsibilities • Conduct monthly CCM and RPM follow up calls. • Review and reinforce individualized care plans. • Assess patient symptoms and identify barriers to care. • Provide patient education within scope of practice. • Monitor and manage RPM alerts according to established protocols. • Escalate patient concerns appropriately to supervising providers. • Support DTC operations as assigned. • Coordinate care with physicians, nurse practitioners, provider offices, and other healthcare professionals. • Maintain accurate, timely, and CMS compliant documentation. • Utilize multiple software platforms including EHRs, CRM systems, and remote monitoring platforms. • Meet productivity, quality, and documentation expectations. • Participate in ongoing training, process improvements, and team meetings. Required Qualifications • Active Licensed Practical Nurse license in good standing. • Active Nurse Licensure Compact (NLC/eNLC) multistate license required. • Must be legally authorized to practice in all compact states covered under the multistate license. • Previous experience in chronic care management or remote patient monitoring preferred. • Comfortable providing care to patients across multiple states as DayaMed expands. • Excellent communication and customer service skills. • Strong computer proficiency and ability to learn multiple software platforms. • Ability to work independently in a remote environment.
