Woundology Clinical Coordinators (LPN) work comprehensively with providers to deliver wound care management for their patients. Woundology is seeking an experienced Licensed Practical Nurse to work 100% Remote – LPN Nurse (LPN) with at least 2 years of direct patient-facing work experience; that thrives in a fast-paced environment, is self-motivated, has impeccable attention to detail, and values the impact they can have on a patient’s healthcare journey. Our home care LPNs will use cutting edge technology and clinical knowledge to provide exceptional care to patients who require one-to-one care and attention in their homes. What's in it for you? Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care. Identify and coordinate visits for their wound care interventions weekly Provide patient education Perform Ultramist Therapy for individual patients in their homes Assist in ensuring timely delivery of services to your patients Resolve patients' questions and create an open dialogue to understand needs and procedures Assist/Manage referrals and appointment scheduling Assist with sending documentation to home healthcare teams
