Job Overview We are seeking a dedicated and skilled LVN or LPN to join our healthcare team. The ideal candidate will support patient care; on a remote patient monitoring platform. They would work directly with the Chronic Care Management Nurse, to help her with the monthly follow-up with CCM patients. They would take direction from the nurse, but work independtly on the platform logging time towards each patients health goals. The candidate must have great communication skills to speak to patients on the required monthly calls. The team member works independantly, but reports to the CCM nurse and myself. Assigned tasks must get done in a timely manner. Having experience in the Cardiology, Internal Medicine or Primary care specialties is preferred. Bilingual is preferred, Spanish speaking preferred, but not required. This role requires a compassionate approach to patient service, proficiency in computers, medical knowledge of Blood pressure and medications. Also familiarity with EHR systems. The Licenced Vocational nurse, or LPN, will play a vital part in ensuring efficient clinic operations and delivering high-quality patient care in a remote job, working independantly while reaching the goals of the program for the office and Heart 2 Heart Medical. They will act as an extention of the physicians care team and conduct themselves as part of the office staff, even though they are a 1099 employee of Heart 2 Heart. Duties • Perform patient monitoring on-line, promoting patient compliance, through communication and documentation of tasks. • Triage patients vital signs for the physician office and CCM nursing team, reporting out of range measuremnts as they are seen, reporting on medication, diet and general monthly care. • Assist with enrolling and unenrolling patients as needed. • Education of how to take a BP reading, or troubleshooting a machine as needed with a patient or their family member. Notifying in-house IT when appropriate. • Understanding the CCM codes for billing purposes and be in line with these codes. 99490 and 99439. Time is in 20 min increments. • The LVN, LPN will be responsible for 20-30 min of the 60 min spent on each patient's care plan throughout each month. Making notes in the charts along the way of what the calls accomplished for the patient. • Maintain accurate documentation within Electronic Health Record (EHR) systems. • Provide patient education on treatment plans, medications, and health maintenance. • Perform administrative support duties such as appointment scheduling and patient follow-up, when needed to help with education and compliance. Skills • Proven experience in clinical settings such as hospital medicine, outpatient clinics, or surgical units. • Strong knowledge of medical terminology, anatomy, physiology, and medications. • Experience with EHR systems and medical administrative support functions. • Ability to work independently, but ask questions when needed to do the job appropriately. • Excellent patient service skills with a compassionate approach to diverse patient populations. • Ability to work efficiently in a remote independant environments with high attention to detail. • Certification as a LVN, or LPN from an accredited program is required; the National certification is necesssary, (if applicable), as the patients you will be caring for are in California and Utah.
