Job Description

We are continuing to grow! Come grow with us!!! Wipro is seeking LPNs who combine excellent nursing and call center skills with the ability to function effectively both as part of a team or on an individual basis to bring their talent to our team.

Requirements

Required Qualifications: Current, unrestricted RN/LPN licensure (include state specific license) 2+ years of clinical experience Proven track record of excellent Clinical record documentation. Preferred Qualifications: Epic Knowledge Has basic knowledge of theories, practices and procedures in a function or skill. Performs routine or structured work. Responds to routine or standard requests. Uses existing procedures and facts to solve routine problems or conduct routine analyses. Depends on others for instruction, guidance, or direction. Exceptional communication skills (written and verbal) 1 year phone experience/remote

Responsibilities

exception to meet KPI 50 calls + a day. Assess patient's health status and recommend care based on clinical judgment and protocols. Identify potential care and/or provider gaps. Coach patients on treatment alternatives Coordinate services and referrals to health programs and community services Assesses and triage immediate prescription requests. Initiate the plan of treatment/care pathway based upon the analysis of information obtained and within scope of practice and certification. Accurately communicate (written/verbal) patient status information to prescribing physician and case manager in timely manner and provide necessary clinical information to Optum pharmacists and other healthcare professionals as applicable. Provide care and services to assigned caseload in an efficient, organized manner. Maintain flexibility in assignments and scheduling.
