This job assists in data collection and implementation of the delivery of health care services. Assists other members of the health care team in providing the highest quality personalized patient care. Maintains a positive work relationship with the health care team. Demonstrates the ability to provide care and service with individuals of all assigned age groups. Engages the patient to maintain optimal health and reduce or minimize acute exacerbations associated with chronic conditions that affect quality outcomes and costs. Oversees the disease registry database. Conducts pre-visit chart review of patients. Coordinates care across the continuum. Participates in quality improvement activities. Performs other related duties as required.
