RPM Healthcare is a fast-growing company with nearly 40 years of patient education experience. Our proprietary software tracks patients’ physiological data (Blood Pressure, Weight, Blood Glucose, Lung Capacity, Oxygenation), identifying dangerous out-of-range values and trends that impact health.

As an RPM 365 Care Coach, you’ll engage patients, track and analyze their data, provide education, and encourage adherence to their care plans. About You: You're a licensed LPN/LVN passionate about healthcare innovation and patient outcomes. Your tech-savvy, comfortable with high-volume phone outreach (125+ conversations a week), and thrive in a fast-changing environment. You’re patient, empathetic, detail-oriented, and a self-starter.

Serve as the primary contact for patients on the RPM365 platform. Assist with device setup and app connectivity. Engage with patients to ensure adherence to care plans. Schedule monthly check-ins and monitor patient data. Educate patients through shared resources. Notify healthcare providers of critical alerts. Attend virtual team meetings for updates.
