In cooperation and collaboration with multiple disciplines, as needed, the RCD Documentation Specialist conducts medical record audits utilizing agency policy and procedures, federal and state regulations and agency education guides. The RCD Documentation Specialist will be responsible for the RCD (Review Choice Demonstration) documentation review and prepare the documents for submission.
Prefer 1 year of medical billing/coding experience in health care setting. Prefer 1 year experience in Homecare Homebase software. Complies with accepted professional standards and practice. Possesses and maintains good physical stamina and mental health. Has completed the agency pre-employment health clearance. Must be a licensed driver with an automobile that is insured in accordance with state and/or Agency requirements and is in good working order. Has satisfactory references from current employers and/or professional peers. Have excellent observation and communication skills. Is self-directed with the ability to work with little supervision. Is flexible and cooperative in fulfilling all obligations. Demonstrates knowledge of appropriate skills for communicating with individual ages of those served. Demonstrates an ability to work as a team member with professional and administrative personnel. Positive attitude. Excellent interpersonal and phone skills. Assertive and quick. Able to multi-task. Accurate and detailed. Must possess a valid state driverâs license and adhere to all state laws while driving. Must possess automobile liability insurance. Must have dependable transportation kept in good working condition. Must be able to drive an automobile in various types of weather/road conditions. Must possess intermediate computer skills. Must be an RN or LPNÂ Home Health experience preferred
Provides a positive work environment by consistently modeling in a positive way, the agency philosophy, mission, values, and standards of care, and providing a professional role model for other staff. Ensures office staff and field staff are educated on process and regulatory changes. Adheres to HIPAA regulations and follows agency protocol maintaining confidentiality and does not improperly disclose patient information. Complies with all agency policies and procedures. Is diligent in activities that ensure advanced proficiency in Homecare Homebase (HCHB) Electronic Health Record (EHR). Actively seeks out training and continuing education in agency processes including participation in offered classroom training and Web-based learning courses. Communicates as appropriate with staff, physicians, referral sources, community, patients, potential patients, caregivers, and others involved in care. As requested by the QAPI Director(s), will assist with the development, implementation and revision, as needed, of materials to meet agency policy and state and federal regulations. Provides ongoing quality documentation education and support to nursing staff according to agency policy. Complies with accepted professional standards and principles. Review all elements of the EHR including but not limited to: Plan of Care, Face to Face Encounter Note, Visit Notes, Medication Profile, Calendar, Coordination Notes, Initial Order, Authorization Information (for non-Medicare patients), Therapy Evaluations and Re- Evaluations, Previous Orders, Physician records, etc. Reviews the EHR of offices assigned to ensure: Documentation supports patients on services meet criteria established by their payer source such as medical necessity and homebound status and ensure documentation clearly indicates the criteria are met. Documentation supports assessing clinicians and ancillary staff providing exceptional patient care by utilizing all elements of the nursing process and/or agency standards of care, and that the level of care and services provided coincide with the patientsâ level of acuity and meet their needs. Documentation supports assessing clinicians and ancillary staff is providing care within their scope of practice and submits quality documentation authenticating appropriate assessment and intervention provided to the patient. Disciplines are following the established plan of care, are compliant with re-assessments within the timelines established by Medicare, providing excellent patient care, providing quality documentation, and meeting the needs of the patient. Reviews discipline only discharges to ensure frequency and goals have been met. Supervisory visits of home health aides are scheduled and performed in person, by required discipline, within timelines in compliance with federal regulations. Continuity of care and follow-up with patient/caregiver and physician(s) as indicated, ensure orders were written as necessary to accommodate patient needs, ensure meds/calendar was updated as indicated, and ensure documentation is thorough.
