Maternity Care Coordinator (RN) – Nevada Location: Nevada (Remote/Field) Required Experience: Maternity, Mother Baby, or NICU care Overview: The Maternity Care Coordinator serves as the primary point of contact for members enrolled in the Mom and Baby Beginnings program, providing assessment, education, and care coordination to support healthy outcomes for mothers and infants.

Telephonically engage, assess, and enroll eligible members into the Mom and Baby Beginnings program. Conduct health and psychosocial assessments and develop person-centered care plans. Educate members on program benefits and available community resources. Collaborate with case managers, providers, and community partners to coordinate services. Document assessments, care plans, and follow-up activities in accordance with CareSource policies. Monitor care plans and adjust as needed to ensure appropriate and person-centered care. Advocate for members and support access to maternity and neonatal care services.
