Multi-State Licensed Nurse Practitioner or Physician Assistant Company: Tara Healthcare LLC Location: 100% remote, United States Type: Independent contractor (1099) Reports to: Medical Director Schedule: Flexible, built around scheduled video visit blocks About Tara Healthcare Tara Healthcare LLC is a telehealth practice that removes the friction from care people usually give up on. We treat opioid use disorder with buprenorphine/naloxone, alcohol use disorder with naltrexone, and metabolic health with GLP-1 therapy, all delivered to the patient's door. We serve patients in 40 plus states, run on the Healthie EHR, and keep the administrative load off clinicians so they can spend their time on patients. Our patients are frequently people who have been failed by traditional care: long waitlists, in-person-only clinics, judgment at the front desk. If low-barrier, evidence-based addiction medicine is the work you want to do, this is the practice for it. What You Will Do • Conduct new patient evaluations and follow-up visits by video across the states where you are licensed, with asynchronous visits available when a patient requests that format • Prescribe and manage buprenorphine/naloxone for opioid use disorder, including induction, stabilization, and long-term maintenance • Prescribe and manage naltrexone for alcohol use disorder • Prescribe and titrate GLP-1 therapy (tirzepatide, semaglutide) for weight management, including dose escalation and side effect management • Order and interpret labs, UDS, and PDMP checks consistent with our clinical protocols and each state's requirements • Document visits in Healthie the same day, using our chart templates • Respond to patient messages and refill requests inside our stated turnaround windows • Coordinate with our pharmacy partners on prescription issues, clarifications, and shipping holds • Escalate complex or high-risk cases to the Medical Director • Help refine clinical protocols and standing orders as our programs grow Licensure Requirements This is a multi-state role. The APRN Compact is not operational, and the Nurse Licensure Compact does not cover APRN practice, so we are looking for clinicians who hold individual active licenses in several states. Required: • Active, unrestricted NP (FNP, PMHNP, AGNP, or equivalent) or PA license in three or more states • An active DEA registration for every state in which you are licensed and intend to see our patients, with Schedule III authority • Current state controlled substance registrations in every state that requires one • National board certification (AANP, ANCC, or NCCPA) in good standing • Clean license history, no board actions, no exclusions from federal healthcare programs • Ability to pass credentialing and a background check Clinicians who maintain licensure in additional states, or who add states over time, will have more patient volume available to them. Experience We Are Looking For Required: • Two plus years of independent prescribing experience as an NP or PA • Direct experience prescribing buprenorphine for opioid use disorder • Comfort practicing telehealth without an in-person safety net • Strong, efficient documentation habits and same-day charting discipline • Clear, non-judgmental patient communication, especially with people who have experienced stigma in healthcare settings Nice to have: • Experience with GLP-1 prescribing and titration • Psychiatric or behavioral health background (PMHNP) • Experience in a high-volume telehealth practice • Familiarity with Healthie or a comparable modern EHR • Experience with collaborative practice agreements in reduced or restricted practice states What Makes This Role Work • Real autonomy. Compensation Per-visit contractor rate, paid monthly: • Synchronous (video) visit: $35 • Asynchronous (message-based) visit: $15 Volume depends on your licensed states and availability. Clinicians licensed in high-demand states can build a substantial recurring panel, since most of our patients are on a monthly or bi-weekly follow-up cadence. As an independent contractor, you are responsible for your own state licensure, DEA registration, controlled substance registrations, board certification, and continuing education. Malpractice coverage is provided through the practice. We review applications weekly and typically move from first conversation to credentialing within two weeks.
