Managing a cohort of heart failure patients, the full-time North Carolina Licensed Telehealth Nurse Practitioner will evaluate patients virtually, create care plans, and optimize medication therapy while working remotely Monday through Friday. Key responsibilities Virtually evaluate patients and assist in creating individualized care plans based on protocols Prescribe and titrate medications according to evidence-based guidelines and company standards Respond to alerts regarding patient vitals and manage clinical status changes within the APP's scope of practice Required qualifications 3+ years as a Board-Certified Adult or Family Nurse Practitioner with experience in chronic care management, specifically in cardiology and heart failure Completion of an accredited NP program with national certification Active licensure in North Carolina and at least one additional state; preference for licensure in multiple states Prior experience managing heart failure patients and prescribing evidence-based therapies Previous telehealth experience is strongly preferred
