This job is a remote/work from home position
Responsibilities
- Responsible for educating, coaching, monitoring, and coordinating resources for individuals with identified health risks.
Provides telephonic and/or web-based outreach to program participants
Provides health coaching, education, clinical advocacy, and navigation assistance for individuals participating in the LNA and/or wellness program(s)
Documents client out-reach, engagement, referrals, and follow-up
Leverages all available tools and resources from the organization’s carriers, third party administrators and other available providers and sources.
Shares the responsibility of maintaining program timelines and resources with the Nurse Advocate program manager, project manager and other key contacts
Assists in the development and presentation of health and wellness-focused information and educational programs
Remains current of health promotion industry trends, products and best practices.
Requirements
- Current unencumbered Registered Nurse Licensure
Minimum of three years nursing experience working with chronic conditions and condition management
Current BLS certification
Competence in Microsoft office, web-based program use, and an ability to readily learn new computer-based skills
Bilingual Spanish speaker strongly preferred
Health or wellness coaching, case management, disease management experience preferred
