This position is responsible for developing, assessing, and coordinating clinical, pharmacy, and care education activities to provide quality, cost-effective healthcare outcomes. The Nurse Care Navigator supports members and providers in identifying prescription drugs on the formulary that align best with the patient’s needs, ensuring optimal medication management, patient education, and care coordination.
