We’re searching for an exceptional Nurse Case Manager for to join our growing team in Cleveland, OH. This position does have the potential to be a hybrid or a remote position after training in our Cleveland, OH office. Position Summary: A Nurse Case Manager for an insurance company helps ensure patients receive appropriate and cost-effective care, acting as a liaison between the patient, healthcare providers, and the insurance company. The Nurse Case Manager assesses patient needs, develops care plans and advocates for the patient's best interests while managing insurance coverage and benefits.
Must be a Registered Nurse (RN) Minimum 2 years’ experience in case management Strong computer and phone skills Working knowledge of insurance/cost savings strongly preferred Comfortable with: Patient outreach & engagement Providing non-clinical health education and consultation Identifying cost savings (e.g., on prescriptions or care episodes) Thorough documentation
Assessment and Planning: Understand the origin of the Case Management Referral (Target Members). Evaluate a patient's medical history, current condition, and support network to understand. individual needs and goals. Engage target members and build a relationship that will support open and constructive dialogue on potentially sensitive topics. Identify barriers to care and provide interventions to address them. Care Coordination: Based on assessment and engagement, create and implement care plans that align with the patient's needs and insurance coverage. Facilitate communication and coordination among the health care team to maximize delivery of appropriate care. Serve as a resource for patients and families. Serve as an advocate for patients and families. Provide education on diagnosis, treatment and self-care. Monitoring and Evaluation: Monitor patients’ progress, health outcomes and make recommendations for changes in the care plan as needed. Conduct regular follow-ups to evaluate care, medical necessity, goal attainment and cost. Documentation and Reporting: Maintain detailed, accurate and confidential patient records in Case Management System. Prepare reports and summaries as required. Document all communications, interventions and outcomes in a timely manner.
