Nurse Case Manager II Address: Work At Home, IL Schedule: M–F, 8:00am – 5:00pm (Full Time)
Critical Information: Requires an RN, LCSW, or LCPC with unrestricted active licensure in IL. Prior experience with case management and IL waiver services is preferred. Verify with a Recruiter or Staffing Manager! Education/Licenses Needed: RN, LCSW, or LCPC with current unrestricted IL state licensure REQUIRED.
MUST HAVE: IL resident who lives in one of the specified counties and is interested in doing field case management with the elderly and individuals with disabilities approved for in-home or nursing facility care. Position will require travel to members' homes up to 50-75% of the time. Candidate must be organized, efficient, and able to work independently. Through the use of clinical tools and information/data review, conduct comprehensive assessments of referred member's needs/eligibility and determine approach to case resolution. Utilize case management and quality management processes in compliance with regulatory and accreditation guidelines.
