Nurse Navigator

Restoration Medicine is a mission-driven virtual pelvic health company redefining how complex pelvic conditions are diagnosed, coordinated, and treated. Based out of the Roux Institute in Portland, Maine, we deliver personalized, multidisciplinary pelvic health care through a fully virtual model. We work with patients navigating pelvic pain, bladder and bowel dysfunction, postpartum and post-surgical recovery, and complex chronic conditions—often after years of fragmented or inadequate care. Our goal is simple but bold: make pelvic health care accessible, coordinated, and deeply patient-centered. As a growing startup, we are intentionally building a small, highly collaborative clinical team where every role has meaningful impact.

We are seeking a compassionate, highly organized Nurse Navigator to serve as a central point of connection for our patients and care team. This role goes beyond traditional nursing duties. You will act as a liaison, guide, and advocate, helping patients access care, understand their plan, and move through our system with clarity and support. You will also assist with intake, care coordination, and evolving operational needs as the company grows. Patient Navigation & Support: Guide patients through their care journey from intake through ongoing treatment Serve as a primary point of contact for questions, education, and care coordination Educate patients and families on diagnoses, treatment plans, and next steps Identify barriers to care and help patients access appropriate resources Care Coordination & Liaison Work: Communicate with Restoration Medicine’s multidisciplinary team to support care plan execution Coordinate with external providers (PCPs, specialists, therapists) as needed Involve caregivers, family members, or social supports when appropriate Intake & Clinical Support: Assist with initial patient intake, history gathering, and documentation Perform nursing assessments and clinical check-ins as appropriate Support telehealth monitoring for higher-risk patients Documentation & Compliance: Maintain accurate, timely documentation of patient communications and interventions Adhere to HIPAA/HITECH guidelines Ensure compliance with regulatory standards (DPH, ERISA, etc.) Assist patients with paperwork, scheduling, and logistics to reduce barriers to care Startup & Growth Support: Take on small, evolving responsibilities as the company grows Provide feedback to improve workflows, patient experience, and systems Collaborate closely with leadership to help shape this role over time
