We are a healthcare technology company building clinical infrastructure for physician-prescribed Remote Therapeutic Monitoring (RTM) and Behavioral Health Integration (BHI) services. Our platform serves Medicare beneficiaries with chronic musculoskeletal conditions and comorbid behavioral health needs across California, Texas, Florida, and Arizona. We combine validated clinical assessments with creative engagement content to drive patient adherence and improve outcomes. We're seeking a Nurse Practitioner to lead clinical program development from the ground up. This is a founding clinical role at an early-stage company. You're building the clinical operation, not stepping into an existing one.
You'll work directly with the founder to design and build all clinical protocols, workflows, documentation standards, and training systems before we go live with patients. This is the infrastructure phase. Once operations launch, this role grows into clinical leadership with direct team management responsibilities. Schedule & Engagement: Type: 1099 Independent Contractor Pay: $50 to $75/hour (monthly retainer) Hours: 10 to 15 hours/week during buildout phase, scaling with patient volume Location: Fully remote Growth: This role is expected to grow into senior clinical leadership as the company scales across all four states
Active Nurse Practitioner license in at least one of our operating states (CA, TX, FL, AZ), with willingness to obtain additional state licensure National board certification (AANP or ANCC) 3+ years of clinical NP experience with exposure to chronic pain, behavioral health, or geriatrics Experience working with Medicare populations and familiarity with value-based care models Hands-on experience building or configuring clinical workflows in a digital health product, EHR, or health tech platform Supervisory experience managing LPNs, RNs, or clinical support staff Self-starting and comfortable working independently in a startup environment with limited structure Preferred: Experience with RTM, RPM, CCM, or BHI programs Prior startup or early-stage healthcare experience Familiarity with PEG, PHQ-9, GAD-7, and Mini-Cog scoring and clinical thresholds Track record of building clinical teams from scratch Multistate NP licensure
Design clinical triage and escalation protocols using validated screening instruments (PEG, PHQ-9, GAD-7) Develop standing order templates for Medical Director approval Build documentation templates aligned with Medicare RTM (98975, 98977, 98980, 98985) and BHI (99484) billing requirements Specify clinical workflow requirements for the technology platform, including intake, daily patient check-ins, escalation routing, and care plan documentation Define enrollment, discharge, and quality assurance processes Team Building & Leadership: Create LPN training materials, competency checklists, and supervision frameworks As patient volume grows, hire, train, and manage the LPN care team Establish chart review processes and clinical quality standards Collaborate with the Medical Director on oversight workflows and escalation management This is a hands-on, entrepreneurial role. We want a clinical partner who is excited to build something.
