Job Description

ROMTech is seeking a Telehealth Nurse Practitioner to join our Chronic Care Management program. This is a part-time contract position (20 hours/week to start), fully remote, supporting patients with chronic conditions using ROMTech’s innovative telehealth and remote monitoring platform. Flexible, remote, potential for more hours as patient volume grows

Requirements

NP with Master’s degree and active state licensure 2+ years clinical experience in chronic care, primary care, or related field Telehealth or remote patient monitoring experience Strong EMR, documentation, and communication skills CCM/RPM experience, multistate licensure, experience with older adults or high-risk patients

Responsibilities

Conduct virtual patient assessments and monthly check-ins Develop and manage individualized chronic care plans Review device and patient-reported data; intervene as needed Provide education on disease management, medications, and therapy adherence Collaborate with ROMTech clinicians and support teams
