ResponsibilitiesCognitive & Brain Health Services • Conduct comprehensive cognitive evaluations via telehealth • Evaluate and manage patients with memory concerns, brain fog, Mild Cognitive Impairment (MCI), dementia, and Alzheimer's disease • Develop individualized treatment and care plans • Monitor cognitive function, behavioral symptoms, and disease progression over time • Educate and support patients and caregivers regarding diagnosis, disease progression, and care strategies • Collaborate with primary care providers, specialists, therapists, pharmacies, and caregiver teams as needed Wellness & Value-Based Care Services • Provide consultations and treatment for blood pressure management • Provide weight management consultations • Evaluate and treat erectile dysfunction • Evaluate and treat common acute illnesses through telehealth visits • Manage medication therapy and ongoing follow-up care • Support Remote Patient Monitoring (RPM) programs • Participate in Chronic Care Management (CCM) services • Collaborate with pharmacy partners to improve medication adherence, chronic disease management, and patient outcomes • Promote preventive medicine and healthy lifestyle interventions that support overall wellness and brain health • Participate in value-based care initiatives designed to improve quality, patient outcomes, and healthcare utilization Clinical & Administrative Responsibilities • Complete accurate and timely documentation within the electronic health record • Participate in quality improvement initiatives and case reviews • Maintain compliance with HIPAA, Medicare, payer, and telehealth regulations • Communicate professionally with patients, caregivers, pharmacies, referral partners, and healthcare providers Required QualificationsRequired • Active Pennsylvania Nurse Practitioner license and national board certification in an appropriate specialty (e.g., Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, or Adult-Gerontology Acute Care Nurse Practitioner) • Additional state licenses welcomed • Master's or Doctoral degree in Nursing from an accredited Nurse Practitioner program • Minimum 2 years of Nurse Practitioner experience • Ability to practice independently via telehealth • Demonstrated clinical skills in patient assessment, diagnosis, treatment planning, prescribing, and management of acute and chronic conditions • Experience in primary care, internal medicine, geriatrics, family medicine, or a related specialty such as community health or chronic disease management • Strong clinical judgment, communication, interpersonal, and documentation skills • Ability to build rapport with patients, collaborate effectively with multidisciplinary teams, and provide clear patient education • Proficiency with electronic health records (EHR), clinical documentation, and digital tools that support coordinated patient care • Commitment to evidence-based practice, patient safety, and adherence to all applicable regulatory and ethical standards • Reliable high-speed internet and a professional remote workspace • Must successfully pass a pre-employment background check • Must maintain an active, unrestricted Nurse Practitioner license • Current DEA registration, or the ability to obtain one if applicable • Must be legally authorized to work in the United States • Must comply with all applicable HIPAA, telehealth, state, and federal regulations • Must be eligible for credentialing with Medicare and commercial insurance payers • Must have no history of disciplinary action, sanctions, exclusions, or licensing restrictions that would prevent credentialing with Medicare or commercial insurance payers or limit the ability to practice telehealth Preferred • 3–5+ years of Nurse Practitioner experience preferred • Telehealth experience strongly preferred • Experience caring for patients with cognitive impairment, dementia, Alzheimer's disease, brain health concerns, or neurological conditions • Experience with Remote Patient Monitoring (RPM), Chronic Care Management (CCM), or value-based care models • Experience working with complex chronic conditions and providing
