Nurse Practitioner — Provider & Practice Owner

Leadership/ManagementNPArizonaFull-timeRemote

Job description

Looking for Partners. This is not a typical NP job post - PLEASE READ BEFORE APPLYING Most telehealth roles pay you per visit and keep the upside. This one is different: we are looking for a nurse practitioner to own and lead the clinical practice behind AZTRT, an established, growing telehealth men's health clinic focused on testosterone replacement therapy. You would own the practice — and we fund the launch. A separate management services organization (MSO) fronts the cost of getting started (advertising, legal and entity setup, software, and technology) and runs everything non-clinical: patient acquisition, marketing, scheduling, billing, and operations, with a proven, ad-driven patient pipeline that books consults onto your calendar. You bring the clinical excellence and licensure; the machine that fills your schedule is already built, running, and paid for. The opportunity • You own most of your practice: majority ownership of the clinical practice entity (structured with healthcare counsel), with full clinical authority and independence • Zero startup capital required from you: we front the cost of advertising, legal and entity setup, software, and technology to get the practice off the ground • You get paid as patients sign up and the practice generates income — per-patient earnings that scale directly with enrollment and volume • Flexible time commitment that grows with the practice: start with a few hours a week of consults and follow-ups, scale as the panel grows • 100% remote telehealth: video consults, async follow-ups, lab review, and ongoing patient management • Launch in Arizona now, with a funded expansion roadmap into Utah and Colorado What you'll do • Conduct initial telehealth consultations, review labs, and determine candidacy for TRT and related men's health therapies • Prescribe and manage treatment plans, including ongoing monitoring, dose adjustments, and follow-up labs • Own clinical protocols, quality, and documentation standards for the practice • Work inside a modern stack (EHR, patient CRM, e-prescribing, async messaging) — the tech is set up for you • Partner with the MSO on growth: patient experience, retention, and new-state launches Must-haves • Active Arizona APRN license (or ability to obtain quickly) in good standing, with certification appropriate to an adult/men's health population (FNP or AGNP) • DEA registration (or eligibility) — TRT involves Schedule III prescribing • Multistate mindset: compact RN license and willingness to obtain APRN licensure in Utah and Colorado (we cover licensing costs and handle the paperwork support) • Tech competence: comfortable running a fully virtual practice — EHR, telehealth platforms, async patient communication. You don't need to be an engineer, but the tech can't slow you down • Expansive attitude: you want to build and own something, not just log visits. Ready to grow with the practice Nice-to-haves • Experience with TRT, hormone optimization, men's health, or functional/preventive medicine • Prior telehealth or direct-to-consumer clinic experience • Existing licenses in Utah, Colorado, or other states Compensation & structure • Per-patient compensation tied to sign-ups and practice income — as the panel grows, so does your pay. Transparent structure discussed in the first conversation • Ownership: majority equity in the clinical practice entity, structured with healthcare counsel (PC/MSO model) • No startup or patient-acquisition risk on you: advertising spend, legal setup, software, technology, and growth operations are all fronted and run by the MSO • Flexible schedule — this can begin alongside your current role and scale into a primary income How to apply Send a short note about yourself, your licensure (states, certification, DEA status), and any hormone/men's health experience to jax@aztrt.com with the subject line “NP Partner — AZTRT.” No formal cover letter needed. We move fast: intro call, clinical conversation, then structure discussion with counsel involved. AZTRT is an equal opportunity organization. Final practice structure, ownership terms, and compensation are formalized with healthcare counsel and comply with applicable state practice and ownership laws. Pay: $5,000.00 - $100,000.00 per year Benefits: • Flexible schedule Application Question(s): • Do you hold an active, unencumbered Arizona APRN license with DEA registration (or eligibility)? • This role pays as patients enroll — no guaranteed salary — with majority ownership of the practice in exchange. What's your honest reaction to that trade? • Describe your experience with TRT, hormone therapy, or men's health. • Can you carry this alongside your current role while the panel builds, and what's your weekly availability? • Are you licensed in any other states besides Arizona? Work Location: Remote
