Job Title: *Nurse Case Manager II Estimated Length of Assignment: 05+ Months With Possible Extension Est. Pay Range: $44.00- $47.00/Hour (USD) MAX Work Type: Remote /Travel (Michigan - Wayne, Macomb, Barry, Van Buren, Kalamazoo, Calhoun, Branch, St Joseph, Cass, and Berrien Counties) Description: This role is working Remotely 50 - 60% travel to assigned nearby areas Mileage will be reimbursed Location: Must be in (Michigan - Wayne, Macomb, Barry, Van Buren, Kalamazoo, Calhoun, Branch, St Joseph, Cass, and Berrien Counties) - Remote /Travel Position Summary: The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes Provide comprehensive healthcare management services to facilitate appropriate healthcare treatment, effectively manage healthcare costs, and improve healthcare program/operational efficiency involving clinical issues. Determines medical necessity/appropriateness. Facilitates optimal outcomes. Identifies and follows through with continuous quality/compliance opportunities. May also include identification of aberrances and initiation of corrective action. Educates/empowers customers to ensure compliance, satisfaction, and promote patient advocacy. Optimizes total costs. Implementation and evaluation of policy based on usage and program directives. Educates/empowers colleagues at all levels to enable decision-making at the most appropriate level
3 years Clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care required. Healthcare and/or managed care industry experience. Case Management experience preferred-- Position requires proficiency with computer skills which includes navigating multiple systems and keyboarding Effective communication skills, both verbal and written. Ability to multitask, prioritize and effectively adapt to a fast paced changing environment Sedentary work involving periods of sitting, talking, listening. Work requires sitting for extended periods, talking on the telephone and typing on the computer. Work requires the ability to perform close inspection of hand written and computer generated documents as well as a PC monitor. Typical office working environment with productivity and quality expectations? Education: RN with current unrestricted state licensure for MI. Case Management Certification CCM preferred
