We support healthcare providers across multiple states through virtual Chronic Care Management (CCM), Principal Care Management (PCM), and Remote Patient Monitoring (RPM) programs, using advanced technology and evidence-based care coordination practices.

We are seeking an experienced and compassionate Registered Nurse (RN) to lead and support our Chronic Care Management (CCM), Principal Care Management (PCM), and Remote Patient Monitoring (RPM) programs across multiple U.S. states, including Texas, Florida, Georgia. The RN will oversee patient engagement, care coordination, documentation accuracy, and regulatory compliance (CMS, HIPAA, state Nursing Board requirements). This is a remote clinical leadership role in a fast-growing virtual care environment. Preferred Location: DFW, TX, USA

Lead and oversee Chronic Care Management (CCM), Principal Care Management (PCM), and Remote Patient Monitoring (RPM) program operations across assigned states Conduct patient outreach, assessments, education, and ongoing care coordination Review, update, and monitor patient care plans in compliance with CMS guidelines Document all patient interactions accurately in the EHR to support billing and audits Monitor RPM device data (BP, glucose, weight, pulse oximetry) and escalate abnormal readings Ensure compliance with HIPAA, CMS billing rules, and state telehealth regulations Collaborate with physicians, care coordinators, tech teams, and internal leadership Track program performance metrics, including engagement, documentation quality, and satisfaction Support readiness for CMS and internal compliance audits
