Registered Nurse

The Registered Nurse plays a key role in delivering patient-centered care for individuals with chronic conditions. This role supports care managers with clinical assessment, triage, and care planning while managing a large, diverse patient population across multiple provider specialties and contracted accounts. The RN will use strong clinical judgment, organization, and adherence to account-specific protocols to ensure continuity of care. This position is also instrumental in helping build and scale Advanta’s RN program as the company grows.

Conduct clinical assessments, chart reviews, and patient triage Provide care recommendations, patient education, and clinical guidance Develop and adjust personalized care plans for chronic condition management Determine appropriate levels of care (ER, urgent care, or scheduled visit) Collaborate with care managers, pharmacists, and providers Ensure accurate documentation and compliance with HIPAA and Medicare CCM guidelines Support program development, protocols, and clinical training This is an opportunity to make a meaningful impact on patient outcomes while helping shape a growing clinical program.
