Registered Nurse | Case Manager | Remote | Michigan | Contract Walker Healthforce is seeking a RN Case Manager for a healthcare client located remotely. This is a contract opportunity. Start Date: ASAP

Lead a multidisciplinary team in delivering holistic, person-centric care management Act as the primary contact for members, caregivers, and providers Utilize the case management process to assess, develop, implement, monitor, and evaluate care plans across the care continuum. Assess members' health, psychosocial needs, and support systems, developing individualized care plans. Arrange necessary resources and coordinate care delivery among support systems, providers, and community agencies. Advocate for members and provide education on health literacy, self-management, medication plans, and nutrition. Monitor and evaluate care plan effectiveness, ensuring adherence to goals. Document interactions accurately to support member management. Prepare members for discharge or transfer, ensuring continuity of care.
