Bridging Gapss
We are seeking an experienced and compassionate Registered Nurse (RN) Consultant to provide clinical consultation and nursing oversight for CLTS clients and other community-based programs. This is a newly developing program, and the RN Consultant will play a key role in supporting program growth, clinical quality, and service development. Compensation is expected to increase over time as the program expands and funding allows. The RN Consultant will serve as a clinical resource to staff, supporting high-quality, compliant, and person-centered care for children, adults, and individuals with developmental disabilities. This role focuses on consultation, guidance, and clinical support rather than direct service delivery.
Current, valid Registered Nurse (RN) license in the applicable state. Experience working with CLTS, waiver programs, or community-based services strongly preferred. Knowledge of developmental disabilities, pediatric and family-centered care, and care coordination practices. Familiarity with behavioral health, trauma-informed care, and crisis response protocols. Understanding of documentation standards, regulatory compliance, and quality assurance processes. Strong consultative, communication, and leadership skills. Ability to work independently while providing guidance and support to multidisciplinary teams. This RN Consultant role is ideal for a nurse who is interested in growing with a new program, contributing to its development, and supporting long-term sustainability. Compensation will be reviewed and increased as the program expands and revenue grows.
Provide nursing consultation and clinical guidance to staff supporting CLTS participants and other program clients. Review and consult on assessments, service plans, and care plans to ensure clinical appropriateness and alignment with CLTS and program requirements. Provide consultation related to medical conditions, health monitoring, and coordination with primary care providers, specialists, and prescribing practitioners. Support staff in identifying and responding to health-related risks, changes in medical status, and complex care needs. Offer clinical recommendations during crisis situations and assist with risk mitigation and safety planning. Collaborate with leadership, care coordinators, therapists, and other multidisciplinary team members to ensure integrated care. Assist with the development, review, and implementation of nursing-related policies, procedures, and best practices. Provide training, education, and ongoing clinical support to staff related to health conditions, documentation standards, and regulatory compliance. Review medical and clinical documentation for quality, accuracy, and compliance with HIPAA, CLTS, and program guidelines. Consult on care for clients with complex needs, including developmental disabilities, chronic medical conditions, trauma-related concerns, and co-occurring mental health diagnoses. Support quality assurance, utilization review, and program development initiatives. Provide consultation through telehealth or remote platforms as needed.
