Registered Nurse (RN) – Home Care / LHCSA

Expert Home Care, a Licensed Home Care Services Agency (LHCSA) serving all five boroughs of New York City, is seeking a compassionate and motivated Registered Nurse (RN) to join our growing team. The RN will conduct in-home visits, complete nursing assessments, develop and maintain Plans of Care (POC), and oversee Home Health Aides (HHAs) and Personal Care Aides (PCAs) to ensure high-quality patient care in compliance with NYS Department of Health regulations.

Conduct initial, reassessment, and supervisory home visits. Develop, implement, and update individualized Plans of Care (POC). Review and maintain MD orders and nursing visit schedules. Provide education, support, and supervision to HHAs and PCAs. Ensure proper documentation and regulatory compliance. Communicate effectively with patients, families, and interdisciplinary teams. Participate in quality assurance and improvement activities. Travel throughout all five boroughs as required for patient visits.
