We are seeking an experienced and empathetic Registered Nurse (RN) to join our team as a Remote Care Manager. In this role, you will engage with patients virtually to assess, coordinate, and monitor their health care needs—playing a vital part in our mission to enhance quality of life through proactive care management.

Conduct remote patient assessments with empathy and professionalism Monitor patients' health status and escalate warning signs appropriately Apply medical terminology knowledge to assess and coordinate care Communicate clearly and compassionately with patients and families Document thoroughly and accurately within multiple computer systems Prioritize daily responsibilities while working independently Provide excellence in patient care and customer service Maintain patient confidentiality and HIPAA compliance at all times
